International Liability Releases and Waivers
These forms are of use to individuals involved with college-associated trips outside of the United States. All participants must complete a Waiver form, the Study Abroad Registration form and the Emergency Response Form. Trip leaders are required to file an itinerary with the Department of Public Safety.
International Module participants should use the waiver below designated for their specific Module.
There are separate forms associated with domestic travel.
This form is intended to be signed by all students, guests, and other non-employees participating in college-sponsored international trips.

Paris ModuleThis form is intended to be filled out by the professor or organizer of any college-sponsored trips and should list all itinerary information.
All study abroad participants (module, independent study, exchange programs, etc.) must complete this online form 20 days prior to departure.
This form provides the College with emergency contact information and critical medical information in the event of an emergency.
