How to Request a Transcript
Please read and follow all instructions to avoid delays in processing your transcript.
The normal processing time is 2-3 business days. Centenary College does not fax transcripts, they must be mailed or picked up.
Transcripts are provided free of charge for the first 25 copies. After 25 copies there is a $2.00 fee for each transcript.
The Family Educational Rights and Privacy Act as Amended protects your educational records. In compliance with this law, Centenary College requires a signed, written request to release your transcript to you or to a third party. E-mailed requests and electronic signatures are not valid. The procedure for obtaining a transcript is listed below.
Print the transcript request form, fill it out and mail it in to the address below, or fax it to 318.841.7266. E-mailed requests are not valid. You may also write a letter using the guidelines below.
- Give your complete name, including any maiden, married, or divorced names, even if you no longer use any of these names.
- Give your social security number. Give your date of birth if you attended before 1987.
- Give the year and semester you last attended.
- List any degree(s) awarded and the year of their conferral.
- Provide the destination of the transcript, including the address and any specific office or person.
- Include any special handling instructions. Double check instructions of graduate schools carefully to see if they want transcripts mailed directly to them or if they want you to include them with your packet.
- Include the special attachments for LSDAS or AMCAS.
- Make sure you sign your request. Computer signatures are invalid.
- Include a daytime telephone number for contact in case of any problems.

Plan ahead to avoid emergencies. Consider the amount of time required for the U.S. mail and for processing.
Students who have attended Centenary since 1999 may check the status of their request by logging on to Banner Web. Transcripts are mailed out the same day that they are printed.
Transcript requests received during registration, grade reporting and graduation periods will be processed immediately following the conclusion of peak time frames.
Additional Information
Requests for financial aid transcripts should be addressed to the Financial Aid Office. (318-869-5137)
Requests for immunization records should be referred to the Student Life Office. (318-869-5117)
Requests for entrance test scores (ACT, GRE, SAT, ect.) should be referred to the testing agencies; they are copyrighted and cannot be reproduced.
Requests for transcripts from other institutions should be referred to the institution of origin. Centenary College is not authorized to copy another institution's transcript. This includes high school transcripts.
Alumni are encouraged to include a personal note to Centenary. These communications are forwarded to the Alumni Office.
