Information for Prospective Students
Eligibility For Services
A person is eligible for accommodations if he/she:
- is otherwise qualified for the program(s)
- is a person with a disability,
- has identified himself/herself to the college through DSS, and
- has presented appropriate documentation regarding the disability to the college as required by DSS (see documentation requirements) and requires accommodation(s).
NOTE: Accommodations are effective after the documentation has been reviewed, approved and the student has completed the DSS registration process. Accommodations are not retroactive. Also, documentation required by DS may not be sufficient at other universities or testing agencies.
Steps for new or prospective students:
1. Complete and submit the Request for Accommodations Form.
2. Review,sign and submit the Confidentiality Statement.
3. Have documentaion submitted about your disability according to the specific documentation guidelines.
4. Set up an appointment to meet with a Disability Services Staff member (call 318-869-5466 or 5424) to review your paperwork and complete additional forms for your individual professors.
