Use this form to request funds and services for long-term facilities investments such as remodeling or significant equipment purchases. While Facilities Services does not approve capital budget requests, they can help with assembling the needed information -- such as equipment and materials costs, project duration, and possible impact on other campus operations -- that could be helpful during the review process.
